Student Identification Policy
The Institute for Medical Aesthetics & Laser (IMAGE) requires verification of student identity as part of the enrollment process. Identification documentation must be submitted before enrollment is finalized and before the student begins attending classes.
Acceptable Government Identification
Students should provide one of the following government-issued photo identifications:
- Driver’s License
- State Identification Card
- Passport
A copy of the identification will be maintained in the student’s permanent file.
Identification for Minor Students
Students under the age of 18 may not yet have a government-issued photo ID. In these cases, IMAGE may accept alternative documentation to verify identity.
Acceptable alternatives include:
- Birth Certificate
- Passport
- School Identification Card
- Social Security Card
- Learner’s Permit
- School record such as a transcript or enrollment verification
If the student does not have a photo ID, IMAGE may require two forms of identification from the list above to verify identity.
Parent or Guardian Verification
Students under the age of 18 must have a parent or legal guardian involved in the enrollment process.
The following documentation is required:
- Parent or guardian government-issued photo ID
- Parent or guardian signature on the enrollment agreement
- Student identification documentation
The parent or guardian is responsible for acknowledging the enrollment agreement and program policies.
Student File Documentation
IMAGE maintains documentation in each student’s permanent record to verify identity and enrollment eligibility. Student files will include:
- Copy of student identification or alternative documents
- Copy of parent or guardian identification (if the student is a minor)
- Signed enrollment agreement
- Any required parental consent forms
These records are maintained in accordance with the Florida Commission for Independent Education (CIE) recordkeeping requirements.
Recommendation for Students Without Identification
Students who do not currently have a government-issued photo ID are encouraged to obtain a Florida State Identification Card through the Florida Department of Highway Safety and Motor Vehicles. State identification cards are inexpensive (typically around $25 or less) and are widely accepted for identification, licensing, and employment purposes.
